Missing detailed report. What exactly is not working? Where does it fail? What did you try to make it work?

installed the plugin and it does not show phone field with flag dropdown.

Did you saw the readme?

How to enable the Country Code Selectbox?
Simply make sure that your form is enabled for HTML5 (find this in the form settings), and properly add a phone field.
